Every new client is warmly welcomed by a member of the Management Team, who’ll go through a thorough care plan and risk assessment to ensure that we know exactly what a client needs, and can deliver a personalised service. We also carry out regular reviews to monitor any change in our clients’ needs over time and to make sure our care at home services are adjusted accordingly.

What is end of life care?
End-of-life care provides the support that is needed towards the end of someone’s life, allowing them to pass in the most peaceful and comfortable way. While some people spend their final days in a hospital, hospice or nursing home, end-of-life care at home has become a preferred option for those who wish to spend their last moments within their own home and surrounded by their loved ones.
End-of-life care is a very personal type of care that is delivered to those in their final days and weeks of their life. At Blakewater Homecare Services, we always keep in mind how sensitive this time is for the individual and for their loved ones, offering both practical and emotional support to all of you.
We have many Support Workers and Snr Managers who have extensive experience with end-of-life care. They will work with you to arrange care to suit your needs and preferences, so that you can rest assured everything is taken care of before, during and after you have passed away.
Your Care and Support is exactly that; yours. Everything from creating the care plan to choosing a meal, is led by you so that you are able to live in a comfortable and dignified manner.
The Right Support Structure
With highly trained Support Workers in the local area, fast-track palliative and End of Life care can be provided at a moment’s notice. We frequently work alongside a number of organisations including hospices, support groups, NHS continuing health care teams, and district nurses. This enables us to offer the right level of home care for both patients and family alongside other elements of your care plan.
